Annual Black Tie Party Order Form

Enter your information and sizes below to order your tuxedo for the Discount Tire Annual Party. The rental price including 7% Indiana sales tax and the $12 Damage & Handling fee is $217.43 and includes shoes and all accessories. Full payments are processed at the time of order.  Your rental order will be available for pickup at the Hyatt Regency Cincinnati any time after 2:00 p.m. on Saturday.  Tuxedo return service will be available at the hotel the next morning between 8:00 a.m. and 12:00 p.m.

Measure around the thickest part of the chest only
Measure around the fullest part of the chest and biceps
Measure around the thickest part of the hips and seat
Usually 10-11 inches longer than your inseam

Your submission will be processed within 72 hours and at that time a non-refundable $40 partial payment will be due. A staff member from the location you have selected will contact you via telephone to confirm the receipt of these measurements and obtain your credit card information over the phone. We do not process credit information electronically for your own protection. All reservations will be made regardless of payment status so long as you are part of an event registered with Louie's Tux Shop. By checking the box below you agree to the following: 1) I will perform a final fitting at the location I have selected no later than 6:00 p.m eastern time. on the Friday prior to the event 2) I understand that my rental must be paid in full before it can be taken from the store 3) I agree to follow all the terms and conditions outlined on my Rental Agreement and at https://www.louiestuxshop.com/rental-terms-conditions 4) I understand that my tuxedo rental must be returned on the next business day to avoid a $20 per day late charge
